Cosmopawlitan Senior Dog Release Form

Grooming procedures can sometimes be stressful, especially for senior pets or pets with health problems, and can expose hidden medical problems or aggravate a current condition(s) during or after the groom.

Senior pets and pets with health problems have a greater chance of injury (including, without limitation, risk of death); these pets will be groomed for cleanliness and comfort, in a manner that will not add to their stress. Cosmopawlitan may determine in its professional opinion (keeping the best interest of your pet in mind) that the only available option is to stop or modify the service for the safety and well-being of your pet.

This release will allow Cosmopawlitan to obtain immediate veterinary treatment for your pet should it be deemed necessary. Cosmopawlitan will do its best to contact the owner/guardian; otherwise we will take your pet(s) to our authorized veterinarian or to the nearest veterinarian that is available. It is agreed that all expenses for veterinary care will be covered by the pet’s owner.

By signing this release, you acknowledge that you fully understand the risks of grooming a senior dog and are allowing Cosmopawlitan to perform the service, while releasing Cosmopawlitan from any injury, loss, liability, claims, damages and/or otherwise which may arise directly or indirectly as a result of the service, and any and all medical problems and/or other conditions that may be uncovered and/or occur during and/or as a result of the service.
